- Connections to community resources (food, transportation, in-home support services)
- Advance care planning conversations and document support
- Discharge planning and transitions of care coordination
- Caregiver support and burnout prevention
- Referrals to long-term care, hospice, or palliative care when appropriate
When to ask for an MSW
- • Family conflict around care decisions
- • Patient is showing signs of depression, anxiety, or isolation
- • Financial pressure is making care choices harder
- • Considering long-term care, hospice, or living arrangement changes
- • Need help connecting to community programs (Meals on Wheels, transportation, IHSS)
